Sentinel Event
A mother brought her child name Tina to Nightingale Community Hospital for a procedure. The pre‐op nurse informed the mother of the time line for the surgery. The mother had some errand to do involving an older sibling and made the nurse aware of it but would return in time to pick up Tina. To stay informed, the mother gave the pre‐op nurse her cell phone number with instructions just in case Tina got out of surgery sooner than expected. Approximately 2 ½ hours later, the mother returned and her child was already discharge 30 minutes earlier to the child’s father without the mother’s consent to the hospital. At this point, hospital security was called, and hospital‐wide child abduction alert was activated. It is apparent that there is a break down in any process that the hospital may have. Since they were able to release the child to someone that claims to be the father, it is clear that there is a flaw on how the hospital handles this process.
The following personnel were interviewed individually during the sentinel event:
1) Tim Blakely, Officer. At 09:00, Officer Tim received a call and was informed that there was potential child abduction. He immediately went to interview the nurse who stated that a child was missing from the facility for approximately 25 minutes. Office Tim questioned why the call didn’t come earlier and believe that if they performed more drilled, the event could have been prevented.
2) Katie Jessup, Registrar. Patient information was entered in the electronic medical record by Katie.
Since she followed instructions on what to ask and enter on the record, she doesn’t feel like she did anything wrong.
3) Anna Liu‐Dilarno, Chief Nursing Officer. Anna is the Chief of Nursing and is in charge of all the nurses in the hospital. Any events that happen on her watch automatically become her responsibility and she

The purpose of this paper is to explain the similarities and differences between “The Sentinel” and 2001: A Space Odyssey. There are many similarities and differences between “The Sentinel” and 2001: A Space Odyssey. The obvious similarity is the crystal pyramid in the story and the monolith in the film. According to Dictionary.com a sentinel is described as a) a person or thing that watches or stands as if watching and, b) a character used to indicate the beginning or end of a particular block of information. The crystal pyramid and the monolith serve as sentinels because in “The Sentinel” the narrator says that the crystal pyramid was one of millions scattered throughout the universe watching over all worlds with a promise of life. This is present in the movie when the apes are basically ignorant in the skills to survive and defend themselves and when the monolith appears their curiosity and understanding changes for the better proving the end of the block of information. ...Hospital Sentinel Event Report Program & Start Date: M.B.A. Health Care Management WGU A1: Sentinel Event Report Minor child, Tina, had a minor operation and as told to child’s mother per Nightingale Memorial Hospital (NMH) pre-op nurse, the operation duration would be 45 minutes plus an additional hour in recovery. Under the instruction of the patient’s mother, the pre-op nurse was to contact her by cell phone if times for release had changed. Mother arrived approximately 2.5 hours later to pick up patient and found that her daughter had been released 30 minutes prior to her arrival. At that point security issued a “Code Pink” (hospital-wide child abduction alert) and local law enforcement was contacted as well. Upon interviewing the patient’s mother, she stated she has custody of the patient and siblings as the parents are currently divorced. Within 30 minutes of mother’s post-op arrival, local law enforcement confirmed the child was in care of her father. Furthermore, the father and child were located at father’s residence awaiting the arrival of the mother. The father was not charged of any crime. Furthermore, NMH CEO assured patient’s mother that the incident would be investigated and changes to policies may be implemented to prevent similar instances in the future.
